If you have moved, you will need to change your address on record with the following Ontario services.

Driver’s licence and/or vehicle permit

Manage your address with an account

Create an account with ServiceOntario that can:

• manage your Ontario products and services
• view a history of your orders
• get notifications and alerts

• change your address online or
• in person at a ServiceOntario centre

To exchange your existing vehicle permit for one showing your new address, you will need to visit a ServiceOntario centre.

A replacement fee may be charged if you do not bring your existing vehicle permit but wish to receive a new one.

Health card

• complete and print a Change of Address Form and mail it to:

Outdoors Card

• change your address online
• by phone, call us at 1-800-387-7011
• in-person at a:
  • participating ServiceOntario centre
  • licence issuer

  Ontario Photo Card

  To change your address on your Ontario Photo Card, you can visit a ServiceOntario centre with your Ontario Photo Card or send a written request with:

  • full name
  • photo card number
  • current address
  • new address
  • signature

  Mail your request to:

  No proof of address is required.

  Ontario student assistance program ( OSAP )

  Businesses

  Follow the steps below to update your business address on ServiceOntario account and with the Ministry of Finance for your business taxes (such as beer tax, wine tax, fuel tax, etc. ).

  1. change your business address using ServiceOntario account
  2. update your business address for business taxes

  Corporations

  You will need to update the address information within 15 days of moving your corporation.

  The completed form must be mailed or delivered to:
